Owner Agreement
I, ____________________________________________, hereby certify that my dog ______________________________ is in good health and has not been ill with a communicable condition within the last 30 days. I further certify that my dog has not harmed or shown aggressive or threatening behavior toward any person or dog. I have read and understand the following:
I understand that I am solely responsible for any harm or damage caused by my dog while my dog is attending Mucho Poocho Doggy Day School (“School”).
I further understand and agree that in admitting my dog to the School, Mucho Poocho’s staff have relied on my representation that my dog is in good health and has not harmed or shown aggressive or threatening behavior towards any person or any other dog.
I further understand and agree that Mucho Poocho Doggy Day School, its staff and volunteers, will not be liable for any problems which develop, provided reasonable care and precautions are followed, and I hereby release them from liability of any kind whatsoever arising from my dog’s attendance and participation at the School.
I further understand and agree that any problems which develop with my dog will be treated as deemed best by the staff and volunteers of Mucho
Poocho Doggy Day School, at their sole discretion, and that I assume full financial responsibility for any and all expenses involved.
I certify that I have read and understand the policies of the School as set forth on the preceding pages and that I have read and understand the conditions and statements of this agreement, including the following:
FEES: Fees for single visits are due and payable at the time of service. Pass Plans must be paid in advance. Days and Hours: Monday through Friday from 7 am to 6 pm. The School is not an overnight facility. Staff goes off duty at 6 pm. Any pet left past 6 pm without prior arrangements having been made will be securely kenneled for the night and an additional fee will be charged.
